NOTE: All Health Link access requests must be processed through the DOM HR Office. Once the documentation for compliance, training and authorization is collected, please submit these items to the DOM HR office. The HR office will submit the compliance certification to SMPH and official authorization request to UWHC. Please follow all directions to ensure each employee receives the proper training for the level of Health Link access requested.
