Q. Who do I go to with questions?

A. First decide if the issue is UW School of Medicine and Public Health, UW Foundation, or UW Medical Foundation.

Q. What do I do with a check for clinical activity?

Q. How do medical staff dues to other hospitals get processed?

A. If you are assigned to do business for the Department of Medicine, the dues should be processed as a section operating expense. Give the notice to your division administrator or section contact for processing. Be sure to sign the form if it requires your signature.

If you are assigned to do business for UW Health, the dues should be processed through UW Health. Send the notice to Sandy Ruef (608) 265-5560 or Roseanne Hepner (608) 890-9962. Both are at 635 Science Drive, Suite 100, Madison, WI 53711. If sending by campus interdepartmental mail, the mail drop code is 9620.

Q. How do I renew my DEA license?

A. Physicians and mid-levels employed by the UW School of Medicine and Public Health are exempt from renewal fees. This fee may be waived whether the renewal is done on-line or via paper. To have the fee waived, do the following:

  1. list your address as Dept of Medicine, 600 Highland Avenue, Madison, WI 53792-2454
  2. check the box to indicate that you are a state employee
  3. in the verification area, list your Section/Division Chief's name and contact information (Section/Division Chiefs, list the Chairman's name and contact information). If you renew via paper, have the appropriate individual sign the form.

The DEA license you will receive will indicate that it is valid for work at “State/Federal Institutions only”. This implies that it is valid for any work you do as part of your UW employment (i.e., outreach activities are covered if done as part of your regular employment). It does not imply that it is only valid at UWHC.

Mid-levels employed only by UWMF are not exempt and the fee must be paid. The original invoice may be submitted for direct payment. If paid by the individual, reimbursement may be requested by submitting a copy of the invoice and proof of payment. Submit the paperwork to your division administrator or section contact for processing. The fee is a section operating expense.

Q. Who do I go to with W-2 questions?

A. If you have a question about your University of Wisconsin W-2, contact the payroll office:

If you need a duplicate of your University of Wisconsin W-2, call (608) 262-5931 and the automated system will talk you through the process or go to the internet address of http://my.wisc.edu. After you LOGIN, click on the work record tab. Scroll down to 'annual tax statements'. Enter the tax year and click on submit if you changed the tax year. Click on 'W2 Statement'. Print if needed.
