A. First decide if the issue is UW School of Medicine and Public Health, UW Foundation, or UW Medical Foundation.

If you need a duplicate of your University of Wisconsin W-2, call (608) 262-5931 and the automated system will talk you through the process or go to the internet address of http://my.wisc.edu. After you LOGIN, click on the work record tab. Scroll down to "annual tax statements". Enter the tax year and click on submit if you changed the tax year. Click on "W2 Statement". Print if needed.
